Our office is located in the Professional Building on the campus of Central Washington Hospital.
Upon arrival to your appointment, please be prepared to provide us with the following information:
- Your insurance card
- Insurance policy holder's date of birth
- Insurance policy holder's social security number
- Any pertinent images
- A list of medications including dosage the patient is currently taking
- Insurance referral if required by your insurance
