Medication Reconciliation Process on Admission  

Admission Reconciliation may be accessed by either the Medication List tab or by the Orders band.

It is recommended that you customize the view to sort the medications by venue. Customizing your view will display the home medications separately from other medication types. 

1. Select the Group orders by: drop-down arrow. Select Venue from the drop-down list. 
2. The Medication List now displays by Venue. Sorted by Inpatient and Documented Medications by Hx.
3. Click the Reconcilation drop-down menu, and select Admission. 
4. To collapse the section, and view all sections, click the black arrow to collapse the View section.  
5. Choose Convert or Do Not Convert by each of the Home Medications.  To complete the Reconciliation, click the Reconcile and Sign button  at the bottom right of the screen.
