Discharge Medication Reconciliation Process 

The Discharge Medication Reconciliation Process is completed on paper.  To access the Meds Reconciliation Report for Discharge:

1. Click on Task from the Toolbar, then click on Reports.  
2. From Reports click on Meds Reconciliation Report for Discharge.  
3. At the bottom of that page, the closest printer will default into the printer field.  Click Print to print this report. To print to a different printer, click the drop-down arrow for a list of printers.  
4. On that printed report, indicate which medications the patient is to continue or discontinue.  Nursing will transcribe that information onto a form for the patient to take with them upon discharge.
