Discharge Readiness Overview

  1. Click on Discharge Readiness from the PowerChart Menu.
  2. The provider is responsible for five of the sections in the Discharge Readiness page: Click to View Screenshot in New Window 
    • Diagnoses = the patient's discharge diagnosis (ses).
    • Medication Reconciliation = creates the patient's dischrage medication list
    • Discharge Instructions = pertinent instructions for the patient post-hospitalization.
    • Discharge Order = communicates to nursing staff that the patient is ready for discharge.
    • Follow-up = indicates when follow-up appointments with providers need to be scheduled.
